Human Function Assessment
Discover Where You Are. Build a Plan for Where You Want to Go
Every journey begins with understanding how your body moves, functions, performs, and recovers. Our comprehensive Human Function Assessment helps identify the root cause of your limitations and creates a personalized roadmap based on your goals.

Why Start Here?
Instead of choosing a service...
Start by discovering what your body actually needs.
Whether you're dealing with pain, looking to improve athletic performance, optimize your health, or accelerate recovery, your assessment helps determine the best path forward.
What We Assess

What Happens During Your Assessment?

Is This Assessment Right For You?
✔ You're experiencing pain
✔ You want to improve performance
✔ You're recovering from injury
✔ You want to optimize your health
✔ You don't know where to start
✔ You're preparing for an event
✔ You want to improve longevity

What Makes This Different?
This is where HFP separates itself.
Most clinics begin treatment immediately.
We begin by understanding how your body functions.
Instead of treating symptoms alone, we evaluate movement, health, performance, and recovery to develop a comprehensive strategy tailored specifically to you.

I recently visited Sheldon while having some intense hip pain. He took the time to explain everything thoroughly and even followed up with me regarding some questions I had after my visit. We went over several exercises in detail and I left not only feeling better, but also understanding WHY my body was doing reacting the way it was. I definitely will be back for some more work and would highly recommend him for anyone looking for support and solid PT.

Sheldon is an excellent physical therapist. Very knowledgeable, efficient, and genuinely caring. He takes the time to listen, explains everything clearly, and is truly committed to his patients’ recovery. Highly recommended!

I had a great experience with Sheldon. I am typically very active, but had developed some low back pain. Sheldon was very patient and explained the pathophysiology of my symptoms, in addition to giving me exercises and stretches to improve my pain.

Frequently Asked Questions
What is the Human Function Assessment?
The Human Function Assessment is your personalized starting point. We evaluate how your body moves, functions, performs, and recovers to identify what's limiting you and develop a customized roadmap based on your goals.
Is the Human Function Assessment the same as a Physical Therapy evaluation?
Not exactly.
A Physical Therapy evaluation focuses specifically on diagnosing and treating conditions appropriate for physical therapy.
The Human Function Assessment takes a broader approach by evaluating movement, health, performance, recovery, and lifestyle to determine the best path forward—which may include Physical Therapy, Performance Physical Therapy, Functional Medicine, Performance Training, Performance Recovery, or a combination of services.
I don't know which service I need. Where should I start?
Start with the Human Function Assessment.
That's exactly why it was created. You don't need to decide which service is right for you—we'll help determine the best place to begin.
What happens during the assessment?
We'll discuss your health history, goals, current concerns, movement patterns, lifestyle, and recovery habits. We'll perform a comprehensive assessment and then review our findings with you to create a personalized plan.
How long does the Human Function Assessment take?
Most assessments take approximately 60–90 minutes, depending on your needs and goals.
What happens after my assessment?
You'll receive a personalized roadmap that outlines the next steps based on your goals and findings. Your plan may include one or multiple HFP services.
Can my roadmap include more than one service?
Absolutely.
Many clients benefit from combining services such as Physical Therapy, Performance Training, Functional Medicine, and Recovery to achieve the best results.
Do I need pain to schedule an assessment?
No.
Many clients come to improve performance, prevent injuries, optimize health, recover more effectively, or prepare for athletic events—not just because they're in pain.
Who is the Human Function Assessment designed for?
It's designed for anyone looking to move better, improve health, enhance performance, recover faster, or build long-term resilience and longevity.
Do you accept insurance?
Human Function & Performance is a cash-based practice. This allows us to provide one-on-one care, individualized treatment, and comprehensive assessments without many of the limitations associated with insurance-based care.
What should I wear?
Wear comfortable clothing that allows you to move freely. Athletic shoes are recommended if movement testing will be performed.
Do I need a physician referral?
In many cases, no. Depending on your situation and state regulations, we can help determine whether a referral is needed.
Can I schedule a specific service instead of the Human Function Assessment?
Yes. Returning clients or individuals who know exactly which service they need can schedule directly. However, for most new clients, we recommend starting with the Human Function Assessment to ensure you're on the most effective path.
