Frequently Asked Questions
How long is an evaluation?
Your initial Physical Therapy Evaluation is approximately 60 minutes. This allows us to thoroughly understand your symptoms, evaluate how your body moves, identify the root cause of your condition, and develop a personalized treatment plan tailored to your goals.
Do I need a referral?
In most cases, no referral is required to begin Physical Therapy. If your condition requires additional medical evaluation or collaboration with another healthcare provider, we'll guide you through the appropriate next steps.
Do you accept insurance?
Human Function & Performance is a cash-based, out-of-network practice. This allows us to provide one-on-one care, longer appointment times, and personalized treatment plans without many of the restrictions imposed by insurance companies.
We can provide a detailed receipt (superbill) that you may submit to your insurance provider for potential out-of-network reimbursement.
How many visits will I need?
Every individual is different. Your plan of care depends on your condition, goals, activity level, and how your body responds to treatment.
Following your evaluation, we'll discuss our recommendations and create a personalized plan designed to help you recover efficiently while working toward long-term results.
What should I wear?
Wear comfortable clothing that allows you to move freely. Athletic or workout attire is ideal, and athletic shoes are recommended if movement or walking assessments will be performed.
Can I come without pain?
Absolutely. Physical Therapy isn't only for injuries.
Many people visit us to improve movement quality, prevent future injuries, address mobility limitations, or prepare for specific activities and sports before pain ever develops.
Can athletes benefit?
Absolutely. We work with athletes and active individuals of all levels—from recreational fitness enthusiasts to competitive athletes.
Our approach focuses on optimizing movement, improving performance, reducing injury risk, and helping you return to your sport stronger and more confident.
What makes Human Function & Performance different?
At Human Function & Performance, we don't just treat where it hurts—we identify why it hurts. Every evaluation begins with understanding how your body moves, functions, and performs so we can address the underlying cause of your symptoms rather than simply treating them. Our goal is to help you move better, function better, perform better, and stay active for the long term.

"I have been with Sheldon for almost 2 years and he is AMAZING !!! I have back issues with L4 & L5, herniated disk, pinch nerve, etc. And I am a "workout-a- holic" !!! I can honestly say this is the best care I have ever received from a physical therapist. He is incredibly knowledgeable and his dry needling treatments have been a game changer for my recovery. He takes the time to truly understand what you are going through, always explains the "why" behind every technique and treatment plan, and tailors every session to your needs. He is patient, thorough, and consistently goes above and beyond. My progress has been remarkable thanks to his skill and dedication. If you are looking for a PT who is both highly skilled and genuinely cares about your recovery - I RECOMMEND him without hesitation. Look no further!!!!"

"Sheldon helped me through a very painful shoulder injury. He explained the root cause in detail from a physiological perspective which helped me better understand how to adjust my posture as we worked through different therapeutic processes. He was very caring - checking in frequently to understand my pain level (during and after therapy) and provided targeted therapy and support. He is also very knowledgeable and thorough connecting movement and opportunities to increase mobility. I am so glad I found this center!"

"Sheldon is very knowledgeable and genuinely cares about the well-being of his clients. I appreciate his holistic approach - looking at the body as a whole, rather than just the injury point - which was a game changer for me. I had been dealing with a knee injury for well over a year and was losing all hope to return to my normal extracurricular activities before I reached out to Sheldon. It was over the phone during our consultation call (!) that he picked up on what the problem could be - something neither my doctors nor other physical therapist had considered. Within just a couple sessions, the pain that I had thought was now just a part of my daily life started subsiding. Sheldon is very thorough in explaining the “why” behind every stretch and exercise, answering all questions, and adapting to your concerns and ability while still ensuring you’re getting all the benefits of treatment, which made me feel like an equal participant in my recovery. I’m now back to doing a lot of the activities I’d had to take a break from with those PT exercises and stretches on hand to help deal with flareups and a better understanding on how to protect and strengthen my body. I definitely recommend working with him for your PT needs!"
